THIS FORM IS REQUIRED FOR ALL CAMPAIGNS.
The results in FF-847 Coupler Compliance Report for the tested family member will be carefully compared to the characteristics and features found in the customer data sheets for the family member named in the compliance report.
The test report should be completed for one member of the device coupler family that is representative of the entire family as a whole. This compliance report must also include data sheets as requested in FF-847. The data sheet(s) should clearly describe the characteristics and features of every member of the device coupler family that is to be considered for registration.

In addition to the forms from this kit, the following items are required for submission:
- Customer Data Sheets(s) for the product(s) described in the Compliance Report.
- JPEG image no larger than 200x200 pixels with a white background to be used on the catalog page for the registered coupler.
NOTE: A considerable number of screen captures from an oscilloscope will be necessary throughout the execution of the Device Coupler Test. During the creation of the compliance report (FF-847), those scope captures MUST be inserted into the document or attached to that page of the report where applicable. Section 5.1 or 6.1 of the compliance report (FF-847) will require a complete data sheet for the device under test to be inserted into the test report or to be attached as an annex. If the required customer data sheet and all required scope captures are not included with the compliance report (FF-847), it will be considered incomplete, and an audit of the test results will not begin. Failure to provide a report that can be audited by the scheduled test report audit date will require a reschedule of the test report audit date.
